Data Deletion Request

Effective date: July 23, 2026

You may ask Pro Vita Care Management Inc. (“Pro Vita”) to delete personal information about you that is no longer required. This page explains how to make a request concerning our website, recruitment or workforce systems, including information associated with WhatsApp communications.

How to submit a request

Email privacy@provita.ca with the subject line Data Deletion Request.

Please include:

  • your full name;
  • the email address or telephone number associated with the information;
  • your relationship with Pro Vita, such as applicant, employee, contractor or website visitor;
  • the system or communication involved, such as WhatsApp; and
  • a brief description of the information you want deleted.

Do not email copies of government identification or other sensitive documents unless we specifically request an approved verification method.

What happens next

Pro Vita may take reasonable steps to verify your identity and locate the relevant records. We will review the request and delete or anonymize information that is no longer required, subject to applicable law and reasonable legal or business requirements. We may contact you if additional information is needed.

Information that may need to be retained

A deletion request does not necessarily require deletion of every record. Pro Vita may retain information where reasonably required for employment or contractor administration, payroll or tax records, contractual obligations, health and safety, legal claims, security, fraud prevention, regulatory requirements, or another purpose permitted or required by law.

If information cannot be deleted, we may explain the reason, subject to legal restrictions. Where appropriate, access to retained information may be restricted or identifying details may be removed.

WhatsApp and third-party records

For a WhatsApp-related request, provide the telephone number used for the communications and enough detail to identify the conversation or records. Pro Vita can address information under its control, but cannot guarantee deletion of information independently retained by Meta, WhatsApp, a telecommunications provider, another recipient, or another organization. Requests concerning information controlled by those organizations should also be directed to them through their applicable privacy or account tools.
